Appointments

Please call in advance for routine office visits. Make follow-up appointments as you leave the office. We make every effort to stay on schedule, although emergencies arise. If we are running late, we will do our best to notify you. We also strive to provide same-day appointments for established patients who need immediate care.

Call the office as soon as possible if you cannot keep your appointment or will be late.

Prescriptions

All prescriptions and refill requests should be made during normal office hours. Please have your pharmacy contact us for the renewal of medications.

We attempt to provide enough refills of your ongoing prescriptions to last until your next scheduled office visit. If you expect to run out of a medication before your next visit, please ask for refills while in the office. 

Allow 2 business days for refill requests to be filled when made outside of an office visit. If you have run out of your medication, your pharmacy should be able to give you a 3-day emergency supply of most medications.

Test Results

You will be contacted by phone or mail for all test results. 

If you have not heard from us within 2 weeks, please call the office.

Financial Policy

All deductibles, co-payments, co-insurance, and any outstanding balances are expected at the time of service. Patients may be financially responsible for payment of all services even if their insurance company does not pay. Patient accounts that are not paid on time are subject to third-party collections and/or legal action.

If we are not participating providers with your plan, we will provide you with a receipt for you to file with your insurance company.

Any check returned from the bank will result in an additional $30.00 charge that will appear on your account. A 25% discount for those patients paying in full at the time of service.

If your insurance carrier has not responded to a claim within 90 days, we reserve the right to formally transfer all associated liability for the claim to the patient/guarantor. Failure to promptly resolve this balance may result in third-party collection efforts and legal procedures. Please keep a close watch for carrier claim payments and contact the insurance carrier or a clinic patient accounts representative at (618) 467-2981 in the event a claim is not resolved within 60 days from the date of service.

We realize that emergencies do arise that may affect the timely payment of your account. If such extreme cases do occur, contact a patient accounts representative at (618) 467-2981.

Always notify our office of any change in name, address, phone or insurance information.

After-hours phone calls are answered by an exchange. The provider on call will be happy to answer your questions or direct you to after-hours care. On-call providers are not allowed to prescribe controlled substances.
